The VPAP Malibu is the latest addition to ResMed’s VPAP family of products. Designed from the ground up to be the most comfortable therapy available for treating unsuccessful CPAP users, VPAP Malibu makes it easy to set up your patients for successful treatment.

Bi-level therapy has diverse applications for multiple types of patients. For instance, some patients who find CPAP difficult to tolerate may benefit from the lowered expiratory pressure of bi-level. This is especially true for CPAP users at pressures of 10 or above. In addition, patients with additional complications such as COPD or other restrictive disorders often require ventilatory assistance, in which case bi-level is a clear treatment choice. The challenge is that many sleep labs find it difficult to achieve a high level of consistency when titrating patients on bi-level therapy.

By combining ResMed’s AutoSet™ algorithm with ResMed’s unique and extremely comfortable breath delivery system, it is now easier than ever to set up bi-level patients for successful treatment. The consistent and appropriate response of the device ensures that each patient will receive the pressure they need when they need it.

In addition to simple setup for unsuccessful CPAP patients and patients requiring ventilatory assistance, VPAP Malibu has features that allow you to customize breath parameters and ensure synchronization and ventilation for your patients. These unique features include TiControl™ and trigger and cycle sensitivity settings, which make VPAP Malibu the most versatile auto-adjusting bi-level device on the market.


ResMed has a long history of innovative algorithms and technologies, and VPAP Malibu combines some of their most important elements. The foundation of the technology in VPAP Malibu is the AutoSet algorithm. This algorithm has been specifically designed to preemptively respond to flow limitations, often preventing airway closure before it occurs. Bliss (2005) compared many auto-adjusting algorithms and showed that ResMed’s AutoSet consistently outperformed the competition, responding and treating respiratory events quickly and consistently. When a patient experiences an event such as a flow-limited breath, the AutoSet algorithm will respond with an appropriate pressure increase on the very next breath. Competitive devices are much slower to respond, leaving the patient vulnerable to subsequent events.

While the AutoSet algorithm automatically adjusts the base pressure level in response to breaths, the VPAP Malibu delivers each breath with a unique pressure waveform that is the most comfortable in the industry. Each breath is delivered with a very natural and physiologic transition from EPAP to IPAP pressure, and then gently slides into a comfortable and adjustable exhalation curve. Because the breath is delivered in such a gentle, natural fashion, patients find the therapy significantly more comfortable than CPAP or traditional bi-level therapy


The VPAP Malibu improves patient care by improving patient comfort while ensuring treatment efficacy. Positive airway pressure therapy does not do any good if a patient does not use it; consequently, ResMed invests in developing the most comfortable bi-level breath so that patients will use their therapy. The AutoSet algorithm enables the Malibu to deliver effective treatment at the lowest required pressure throughout the night. For instance, if a patient requires 4 centimeters of additional pressure while sleeping on his/her back rather than his/her side, that extra pressure is delivered only when the patient is on his/her back. This makes the VPAP Malibu extremely comfortable to use.

This device was also designed with our HME partners in mind. The default settings were chosen so that many patients can be treated with a one-touch setup. This makes the device extremely simple to place on patients. Additionally, the automatically adjusting pressure of the AutoSet algorithm reduces the need for clinicians to make manual adjustments to pressure as their patient’s pressure needs change over time. Ultimately, this saves our partners time and money. The VPAP Malibu works with ResScan™ software and tracks patient efficacy and usage data, providing important information to the caregivers and physicians.


With the ability to deliver therapy in auto or standard spontaneous bi-level modes, we expect the VPAP Malibu to become the bi-level device of choice for our HME partners. It has all of the features and performance of our popular VPAP III devices, along with the new comfort features described above. Once patients sleep with a VPAP Malibu, they will continue to demand the high level of comfort that it provides. ResMed is setting a new standard for comfort in the market.
